מנוי הרוצה להביא תלונתו בפני ועדת הערר יגישה בכתב במען שתציין ועדת הערר בהודעה שתפורסם בעתונות, ויצרף לפנייתו מסמכים המתיחסים לתלונתו וכן העתק מהודעת המנהל הכללי על דחיית פנייתו אליו או על העדר תגובה תוך 90 ימים מיום הגשתה כאמור בסעיף 4(א).
