2. הגשת בקשה
(א)בקשה תוגש למועצה כשהיא מודפסת וערוכה לפי סדר העניינים והסעיפים שבשאלון.
(ב)מבקש יפרט בבקשתו באופן מלא ומדויק את כל המידע הנדרש, ויצרף לבקשתו את כל הנספחים הנדרשים.
