19. צורת הזמנה ותכנה
הזמנת עד בכתב תהיה חתומה ביד יושב ראש ועדת המשמעת ותכיל פרטים אלה:
(1)הקובל;
(2)שם הנאשם;
(3)שם המוזמן ומענו;
(4)המקום והמועד שבהם עליו להתייצב להעיד או להגיש אותם מסמכים שיפורטו בהזמנה;
(5)תמצית ההוראות הנזכרות בסעיף 22(ב) לחוק.
