44. צורת בקשה להתלות רשיון
בקשה להתלות רשיון לפי סעיף 27 לחוק תכיל פרטים אלה:
(1)שם המבקש;
(2)שם החוקר הפרטי שהבקשה מתייחסת אליו, מקום עבודתו ומענו;
(3)הבקשה להתליית הרשיון ונימוקיה.
