28. פרטים אישיים של כשיר לטיפול לפי דיני חוץ בתנאים מסויימים
מטפל יחיד שהיה כשיר לייצג תובעים באחת המדינות שהעניקה זכויות תביעה אילו היה תושב או אזרח של אותה מדינה – ימסור פרטים לפי האמור בטופס 2 שבתוספת.
