30. פרטים אישיים של הממשיך בטיפול מלפני 1957
מי שטיפולו בתביעות אינו אלא המשך וגמר באותן התביעות שהוגשו על ידו או מטעמו לפני יום י״ג בשבט תשי״ז (15 בינואר 1957) לבית המשפט או לרשות המוסמכת בחוץ לארץ – ימסור פרטים לפי האמור בטופס 4 שבתוספת.
