14. רישום פרטים אישיים
בשעת קבלת חולה יירשמו לגביו פרטים אלה:
(1)שם מלא ושמות ההורים;
(2)מספר תעודת זהות או דרכון;
(3)שם, מען ומספר טלפון של קרוב משפחה אחד לפחות;
(4)שנת לידה וארץ לידה;
(5)מין;
(6)דת;
(7)אזרחות;
(8)מצב משפחתי;
(9)מען קבוע או אחרון ומספר הטלפון בו.
