2. הודעה על מען בישוב
הודעה על המען בישוב ועל שינוי מען לפי סעיף 2(א)(11) לחוק (להלן – הודעה) תכלול פרטים אלה:
(1)שם הישוב;
(2)שם הרחוב או מספרו, ואם אין שם או מספר לרחוב – שם הרובע, השכונה או השיכון;
(3)מספר הבית, ואם אין מספר לבית – שם הבית.
