2. היתר למוסד
(א)בקשה להיתר למוסד יגיש מנהל המוסד למועצה בכתב; בבקשה יצוין שם הרופא הווטרינר המועסק בו כאמור בסעיף 12(2) לחוק, ויצורף אליה קובץ הנחיות כנדרש בסעיף 12(1) לחוק.
(ב)המועצה תיתן היתר למוסד, לאחר שאישרה את קובץ ההנחיות שצורף לבקשה, ולאחר שהתקבל אישור וטרינר המועצה כי בביקור שערך במוסד מצא כי תנאי החזקת בעלי החיים במוסד עומדים בדרישות סעיף 4.
