4. טופס הרישום
(א)רוקח המספק סם למחלקה בבית חולים, יצרף לכל סם שהוא מספק טופס רישום, ערוך לפי טופס 2 שבתוספת (להלן – טופס הרישום).
(ב)טופס הרישום יימסר למחלקה אך ורק מבית המרקחת.
