3. עריכת הדוח על ידי קופת חולים
(א)קופת חולים תערוך את הדוח בהתאם למידע המצוי בידה, ולא תאסוף מידע מן המבוטח או מכל גורם אחר לצורך זה בלבד.
(ב)קופת חולים לא תשמור את הדוח, כולו או חלקו, במאגר מידע.
