1. הודעת עובד
עובד המבקש להיעדר מהעבודה מכח סעיף 7(ד1) לחוק, ימסור למעבידו הצהרה על כך לפני תחילת ההעדרות מהעבודה, חתומה בידו ובידי בת זוגו – ובנסיבות האמורות בסעיף 7(ד1)(2) לחוק – בידו בלבד, לפי הטופס שנוסחו מובא בתוספת.
