3. מטופל הנזקק לטיפול כרוני
לעניין הוראות אלה, מטופל הנזקק לטיפול כרוני הוא כל אחד מאלה:
(1)נזקק לטיפול באותה תרופה או תרופות, במשך תקופה רצופה של חצי שנה לפחות;
(2)מקבל טיפול תרופתי לאחת או יותר מן המחלות המנויות בטור א׳ בתוספת;
(3)מטופל במנות אחזקה חודשיות בתרופה אחת לפחות מן התכשירים הכלולים באחת או יותר מקבוצות התכשירים שבתוספת.
