5. מתן הרשאה
(א)הרשאה אישית ייתן המנהל הכללי של משרד הבריאות, המנהל הרפואי הראשי של קופת חולים – למוסמך שהוא עובד אותה קופת החולים או מנהל מוסד רפואי – למוסמך שהוא עובד אותו מוסד; הרשאה כאמור לא תינתן אלא לבעל ותק של חמש שנים כאח מוסמך או כאחות מוסמכת, לפי העניין, שעבר הכשרה כאמור בסעיף 6 והרשום בפנקס כאמור בסעיף 6(ג).
(ב)הרשאה אישית יכול שתהיה כללית, ויכול שתתייחס לקבוצה או קבוצות מוגדרות של חולים או תרופות, הכל בכפוף להוראות אלה.
