2. בקשה למתן רשיון
(א)בקשה למתן רשיון תוגש למנהל לפי טופס שיקבע.
(ב)הבקשה תיחתם ביד המבקש לאחר מילוי הפרטים הנדרשים בטופס.
(ג)לבקשה יצורפו תעודות לאימות הפרטים בטופס.
