3. הגשת בקשה
(א)בקשה לרשיון תוגש למפקח בטופס שקבע.
(ב)מבקש רשיון ישיב תשובות מלאות ונכונות על כל שאלה שבטופס ועל כל שאלה נוספת של המפקח בקשר עם הבקשה.
(ג)מבקש רשיון ימציא למפקח, לפי דרישתו, מסמכים, תצלומים, תכניות, הצהרות ופרטים נוספים.
(ד)בקשה לחידוש רשיון דינה כדין בקשה לרשיון.
