7. הגשת ערעור
הערעור יוגש ליושב ראש על טופס מיוחד שייקבע על ידי הרשות המוסמכת בצירוף החלטת הועדה הרפואית שעליה מערערים. העתק מהערעור יישלח לרשות המוסמכת.
