5. כתב ערעור
ערעור בכתב יוגש בארבעה טפסים, ייחתם על ידי המערער או בא כוחו, ויפורטו בו:
(1)שמו ומענו של המערער;
(2)שמו ומענו של המשיב;
(3)ועדת הערעור שעל החלטתה מערערים;
(4)יום מתן ההחלטה, ואם נקראה בפני המערער או בא כוחו או לא;
(5)המועד שבו נמסר למערער העתק ההחלטה אם נקראה שלא בפניו או בפני בא כוחו;
(6)המספר הסידורי של הערעור בועדת הערעור;
(7)הנקודה המשפטית שבה מערערים על ההחלטה.
