3. פרטי הערר
בכתב הערר יפורטו –
(1)שמו של העורר, תארו, משלח ידו ומענו;
(2)תכנה של ההחלטה;
(3)התאריך שבו ניתנה ההחלטה ושבו היא נמסרה לעורר;
(4)נימוקי הערר, לרבות נימוקים משפטיים, כל נימוק בפסקה מיוחדת ובקיצור;
(5)הסעד המבוקש.
