3. בקשה להיתר
(א)קבלן מורשה המעונין לייצא חומר רביה, יגיש למועצה בקשה להיתר לפחות חמישה ימים לפני התאריך המיועד של המשלוח ליצוא.
(ב)בקשה להיתר יש להגיש לפי טופס שקבעה המועצה; קבלן מורשה ימלא בטופס פרטים מלאים ונכונים וימסור למועצה פרטים נוספים לפי דרישתה.
