ביקש אסיר זר לעבור למדינת אזרחותו בשל מצב בריאותו יוותר בכתב על החיסיון בקשר לתיקו הרפואי וימלא בטופס מתן הסכמתו את הספח המיועד לכך ויחתום עליו.
