הפרטים המתוארים בסעיף 22(א)(3) לחוק, יימסרו בידי הנושה לערב יחיד, במסמך הגילוי כאמור בתקנה 1, ובסמוך לאחר מתן האשראי לחייב, ימסור אותם הנושה לערב בצורה של לוח תשלומים.
