3. חידוש אישור
(א)בעל בית עסק יגיש לממונה בקשה למתן חידוש האישור הערוכה לפי הטופס שבתוספת הרביעית ויצרף לה אישור מרואה חשבון כי לעוסק אין חובות לפי חוק מס ערך מוסף ודוחותיו מוגשים בזמן, ואישור ניהול פנקסי חשבונות ורשומות לפי חוק עסקאות גופים ציבוריים.
(ב)בקשה לחידוש אישור יכול שתוגש עד שלושה חודשים לפני תום תוקפו של האישור.
