4. הסכמה למינוי
(א)לא ימונה תומך אלא אם כן מקבל ההחלטות הסכים למינויו, לרבות לעניין משך התקופה ותחומי התמיכה.
(ב)לא ימונה תומך אלא אם כן נתן את הסכמתו למינוי, לרבות לעניין משך התקופה ותחומי התמיכה; בהסכמה כאמור יציין תומך מתנדב או תומך מקצועי לכמה מקבלי החלטות הוא משמש תומך באותה עת.
