4. הודעת ויתור על מכסה ובקשה לפיצוי
(א)הודעת ויתור על מכסה לפי סעיף 34(ד)(1) לחוק ובקשה לפיצוי בשל ויתור על המכסה (להלן – הבקשה), יוגשו למועצה בידי בעל המכסה על גבי הטופס שבתוספת.
(ב)בתוך שבוע מתחילת כל רבעון, המועצה תעביר אל ועדת הפיצויים את הטפסים שהוגשו לה לפי תקנת משנה (א) ברבעון שקדם לו.
