4. בקשה לאישור מיוחד
עמית רופא המעוניין לקבל הרשאה אישית לפי אישור מיוחד שייתן המנהל כאמור בסעיף 17ו(ג)(3) לפקודה, יגיש בקשה לפי תקנה 2, זאת לאחר שמנהל מוסד רפואי הגיש בעבורו בקשה לקבל אישור מיוחד, באמצעות המערכת המקוונת, הכוללת את הפרטים המנויים בחלק א׳ לתוספת הראשונה בצירוף הנימוקים לבקשה; החליט המנהל לתת אישור מיוחד, ימסור העתק ממנו לעמית הרופא אשר בעבורו הוגשה הבקשה, הכול באמצעות המערכת המקוונת; על הרשאה אישית לפי אישור מיוחד לא תחול תקנה 3.
