7. מסמכים שיש לצרף לבקשה לחידוש היתר להפעלת מיתקן גז
לבקשה לחידוש היתר להפעלה של מיתקן גז ספק הגז יצרף את כל אלה:
(1)אישור של בעל רישיון עבודת גז כי המיתקן ואביזריו נבדקו לפי דרישות ההיתר להפעלת המיתקן ותקן ישראלי החל על המיתקן, או לפי צו המחסנים, לפי העניין, והם מתאימים לדרישות כאמור;
(2)הצהרה של בעל רישיון עבודת גז כי לא חל שינוי יסודי במיתקן מאז שניתן ההיתר הקודם;
(3)מסמכים כמפורט בתקנה 4(2).
