Section 9 of תקנות העתיקות (חצרים), תשי״ט–1959, פנקס.
9. פנקס
(א)פנקס יכלול את –
(1)שם הנוטר;
(2)מספר תעודת זהות שלו;
(3)מספרו כנוטר;
(4)תאריך מתן הפנקס וכן תאריך חידושו האחרון של הפנקס;
(5)תצלומו של הנוטר.
(ב)פנקס ישא את חתימת המנהל ואת חתימת הנוטר.
