1. הצהרת עובד
תיקון: תשס״ט
(א)עובד שנעדר מעבודתו מכוח סעיפים 1, 1א או 1ג לחוק בשל מחלת ילד, ימסור למעבידו הצהרה חתומה בידו וביד בן זוגו, או בידו בלבד, הכל לפי העניין, לפי הטופס שבתוספת הראשונה.
(ב)עובד שנעדר מעבודתו מכוח סעיפים 1ב או 1ג לחוק, לצורך מתן סיוע אישי לאדם עם מוגבלות, ימסור למעבידו הצהרה חתומה בידו, לפי הטופס שבתוספת השניה.
