1א. פרטי זיהוי
תיקון: תשמ״ח־2, תשפ״ב
בכל פניה לבית המשפט, יציין המבקש את הפרטים הבאים של הנאשם:
(1)שם פרטי ושם המשפחה;
(2)מספר זהותו וסוג התעודה ממנה נרשם המספר;
(3)המען, ואם הנאשם נמצא במשמורת, מקום כליאתו, ולצדם – המיקוד;
(4)מספר תיק בית המשפט;
לענין תקנה זו, ”נאשם“ – לרבות עציר, מערער, נידון ואסיר.
