מסמך שיש לערכו לפי אחד הטפסים שבתוספת הראשונה יהא ערוך בשינויים המחוייבים לפי הנסיבות ואין חובה שיהא העתק מילולי ממנו; היתה בטופס דרישה להמצאת פרטים, רשאי בית המשפט או הרשם, לצורך הדיון שלפניו, לצוות על מתן פרטים נוספים כפי שיצויין בצו.
