הודעת שוטר ודו״ח בוחן תנועה לענין סעיף 27 לפקודה, יהיו לפי טפסים שבתוספת החמישית או בנוסח דומה להם; טופס כאמור יכול שיהיה חלק מטופס אחר או בנפרד.
