מי שניתן לו אישור ימציא למנהל, בהתאם לדרישתו, ידיעות, מסמכים וראיות אחרות בקשר לביצוע התכנית המאושרת, לקיום תנאי האישור ותנאי היתרים ולקביעת שיעורי ההטבות.
