3. הגשת בקשה למתן רשיון
תיקון: תשמ״ח
(א)מי שהתקיימו בו הוראות תקנה 2, רשאי להגיש לממונה בקשה למתן רשיון בטופס שקבע הממונה למתן רשיון מורה דרך.
(ב)לבקשה יצורפו שני צילומי דרכון של המבקש וכן אישורים נוספים כפי שידרוש הממונה.
