4ג. בקשות לרשות מוסמכת
תיקון: תשכ״ט, תשל״ז, תשמ״א, תשע״ב
התובע תגמול מוגדל לפי הכנסה לפי סעיף 4א יגיש בקשה לרשות המוסמכת בצירוף ראיות הדרושות להוכחת תביעתו כפי שנקבע בתקנות, ולא ישולם תגמול מוגדל לפי הכנסה בעד תקופה העולה על 120 יום לפני הגשת הבקשה לתגמול כאמור.
