11. רשימת מסמכים

(א)התובע ירשום בכתב התביעה או ברשימה המצורפת אליה, מה הם המסמכים שעליהם הוא נסמך בתביעתו, בין שהם ברשותו ובין שאינם ברשותו.
(ב)מסמך שרשם התובע כאמור בתקנת משנה (א) והוא ברשותו, יצרף העתק או תצלום שלו לכתב התביעה; אם איננו ברשותו – יציין בידי מי הוא נמצא לפי מיטב ידיעתו.
