12. טפסי הזמנה

(א)הוגשה תובענה לבית דין אזורי, תומצא מטעם בית הדין לכל נתבע הזמנה לדין חתומה ביד הרשם או פקיד של בית הדין האזורי, יחד עם עותק של כתב התביעה.
(ב)ההזמנה לדין כאמור בתקנת משנה (א) תהיה ערוכה על פי טופס 1.
