תיקון: תשנ״ב [61יג]
בכתב תביעה שעילתו בחוק הביטוח הלאומי [נוסח משולב], התשכ״ח–1968 (להלן – חוק הביטוח הלאומי) או בחוק אחר שהמוסד לביטוח לאומי פועל על פיו, יצויין מספר זהותו של התובע ויצורפו לו העתקי כל החלטות המוסד לביטוח לאומי שבגינן מוגשת התובענה.
