23ד3. בקשה לקבלת מידע על חוב מזונות במסלול המקוצר
תיקון: תשע״ט
בבקשה לקבלת מידע לפי סעיף 7ב3(ג) לחוק על חוב מזונות שמגיש זוכה בתיק המתנהל במסלול המקוצר לפי פרק א׳1 לחוק, יצהיר הזוכה כי –
(1)המידע המבוקש נדרש לו לשם החלטה אם המשך ניהול התיק יהיה במסלול המקוצר;
(2)הוא לא יעשה במידע כל שימוש לתכלית אחרת מזו האמורה בפסקה (1) ולא יעבירו לאחר.
