37ב. חובת דיווח
תיקון: תשנ״ב
הגיש בעל אישור בקשה לחידוש האישור, יצרף לבקשה דיווח בכתב על כל אחד מאלה:
(1)הוגש נגדו כתב אישום;
(2)נפתחו נגדו הליכי הוצאה לפועל;
(3)הפך לקוח מוגבל כמשמעותו בסעיף 2 לחוק שיקים ללא כיסוי, התשמ״א–1981;
(4)ניתן נגדו צו הגנה לפי חוק למניעת אלימות במשפחה, התשנ״א–1991;
(5)כל ענין העלול ליצור ניגוד עניינים עם חובותיו כבעל תפקיד.
