שונה שמו של מורשה לעסוק בריפוי שיניים מכל סיבה שהיא, לפי חוק השמות, תשט״ז–1958, יודיע בכתב על כך למנהל באמצעות מנהל המחלקה ולרופא המחוזי, תוך שלושים יום לאחר תאריך השינוי, בצרוף תעודת משרד הפנים על שינוי השם.
