5. רישום שינוי שם
נוכח המנהל כי שמו של מורשה לעסוק בריפוי שיניים שונה כדין ונמסרה הודעה כאמור בתקנה 4, יורה לשנות את שמו של המורשה לעסוק בריפוי שיניים ברשיונו ובכל רישום אחר הנעשה מטעם משרד הבריאות.
