8. טופס בקשה על רשיון ועל תעודת־היתר וטופס רשיון ותעודת־היתר
תיקון: תש״ח
(1)כל בקשה על רשיון או על תעודת־היתר תהיה בטופס שיקבעהו המושלט.
(2)כל רשיון ותעודת־היתר יהיו באותו טופס ולמשך אותה תקופה וכפופים לתשלום אותם דמים, שיהיה שר הבטחון עשוי לקבעם, ויהיו כפופים לאותם דברים ותנאים, שיהיה הוא עשוי לראות לנכון להטילם, ובהתאם להם.
