12. הודעה על גמר הכשרה ודו״ח
תיקון: תשמ״ח
(א)המאמן יגיש לנציב בתום תקופת ההכשרה אצלו, הודעה על גמר תקופת ההכשרה של המתמחה אצלו, כמפורט בתוספת השניה, וכן דו״ח המפרט את עיסוקי המתמחה בתקופת ההכשרה.
(ב)הנציב רשאי לדרוש מן המאמן ומן המתמחה הסברים בקשר לדו״ח.
