6. אישור
חייב שניכה מס כאמור בתקנות אלה ייתן למקבל על פי בקשתו, בסוף שנת המס ולא יאוחר מיום 20 במרס של כל שנה, אישור לגבי שנת המס הקודמת, לפי טופס 0857 על הסכום ששילם ועל המס שנוכה ויפרט את הפרטים כאמור בתקנה 3 וכן כל פרט אחר שברשותו שידרוש הנציב.
