21. בקשה להחזר הוצאות
(א)לא שילמה המדינה המבקשת למוזמן את ההוצאות האמורות בסעיף 26 לחוק, רשאי המוזמן להגיש בקשה לרשות המוסמכת, להחזר הוצאות כאמור בסעיף 26(ב) לחוק.
(ב)לבקשה יצרף המבקש פירוט ההוצאות והקבלות הנוגעות לענין.
(ג)בקשה כאמור בתקנת משנה (א) תהיה לפי טופס 11 שבתוספת.
