6. הודעה על מען
תיקון: תשמ״א
רוקח ועוזר רוקח יודיע בכתב על מען מגוריו, ועל כל שינוי בו, למנהל באמצעות מנהל המחלקה למקצועות רפואיים במשרד הבריאות (להלן – מנהל המחלקה) ולרופא המחוזי, תוך ארבעה עשר יום מיום שניתן לו הרשיון או מיום השינוי, לפי הענין.
