1. בקשה לקבלת שירות משפטי
המבקש שירות משפטי על־פי החוק (להלן – שירות), יגיש בקשה ללשכת הסיוע המשפטי שבתחום פעולתה נמצא מקום מגוריו; הבקשה תהא ערוכה בטופס שיימסר חינם בכל לשכת סיוע משפטי.
