בעד רישום מרפאה החייבת רישום כאמור בתקנה 4, לרבות רישום זמני לפי תקנה 6, או בעד חידוש רישום כאמור בתקנה 8, תשולם אגרה בסכום כמפורט בפרט 30 בתוספת השנייה לתקנות אגרות בריאות, התשמ״ט–1989.
