ההודעה וכתב ההתחייבות הנדרשים לפי התקנות 4 ו־6 של תקנות אלה יהיו ערוכים בטפסים שבתוספת לתקנות אלה, או בטפסים אחרים, ככל אשר יורה המנהל מזמן לזמן.
